Answer the following:

• Please summarize the expected outcomes for this project and to what extent they have been achieved.
• What have been the main accomplishments of the project to date and how have they been achieved?
• How many individuals have been served through this project?
• The Foundation recognized that circumstances change, possibly affecting project implementation. What, if any, difficulties have you encountered? Why did they occur, and what refinements or plans have been made to overcome them? Please indicate activities that are behind schedule or not yet begun and any changes in project plans.
• What have been the most challenging or surprising aspects of the project? Have there been any unexpected outcomes?

Budget Information

• Total amount of funds spent to date:
• Explanation of how funds were used (purchases, personnel, etc.)

Please submit the responses to these questions to hodgesfoundationgrant@gmail.com by December 1st of each year and prior to submitting a new grant application if previously funded and seeking additional funding.
